“We take your insurance” is not the same as in network
An office may submit claims to a plan without having a contracted network rate. Ask the precise question: “Are the treating dentist and this location in network for my exact plan?” If the answer is unclear, call the number on your card.
Benefit verification is a snapshot, not a promise
The insurer makes the final claim decision after treatment. Deductibles, remaining annual maximum, waiting periods, frequency limits, alternate benefits, exclusions, and information from other recent claims can change the result.

Weekend verification may be limited
An office can be open when an insurer’s service line is closed. Ask whether you must pay a deposit or self-pay amount, whether the claim will still be submitted, and how any resulting credit is refunded or applied.
Health First Colorado follows a different process
Medicaid eligibility, provider enrollment, covered benefits, and authorization rules are different from commercial PPO insurance. Use the official Health First Colorado resources and directly confirm that the dental office is accepting members for the needed service.
If the claim is denied or different from the estimate
Ask for the explanation of benefits, procedure codes, clinical notes or images used, and the appeal process. Compare the insurer’s reason with the office’s estimate before assuming either side made an error.

Common questions
Do I need preauthorization for an emergency dental visit?
Plans vary. An exam may not require it while a later procedure might. Ask whether waiting for authorization is clinically safe.
What if I forgot my insurance card?
The office may be able to search with the policyholder’s name, date of birth, employer, and member ID, but bring a digital card or call the plan if possible.
Will insurance cover a same-day extraction or root canal?
It may cover part of a clinically necessary service, but network rules, deductible, annual maximum, exclusions, and authorization requirements affect the patient portion.
Can I see an out-of-network emergency dentist?
Usually you can choose one, but benefits and balance billing may differ. Ask for the self-pay amount and out-of-network estimate before treatment when the situation safely allows.
